Discharge Planning

Companion Program
If the patient will require a caregiver at the time of discharge, a companion program is available. This allows the caregiver to stay at the Rehabilitation Center and, with supervision, assume responsibilities of care to the patient. This helps identify areas of care that might require further caregiver training.

Independent-Living Room
Our Independent-Living Room allows a patient who may be discharged home without caregivers to function within a home environment, so that problems can be identified while supervision is available.

As the patient prepares for discharge, a home visit may be made. This visit allows the team to assess the home setting and provide recommendations that can make the transition from hospital to home easier and safer.

When the patient is ready to be discharged, the team works with the patient and family to make the transition home and back into the community a positive experience. Discharge instructions are tailored for each patient’s needs. Team members also help make arrangements for nursing services, medical equipment or outpatient services near the patient’s home. Outpatient therapies or home health provide follow-up care to help patients further develop the skills they need to ensure a higher quality of life.
