Rehabilitation Center - Brady Building

Admission Process

The patient’s primary care physician makes most referrals to St. John’s Rehabilitation Center. Other health professions such as discharge planners, social workers, therapists or insurance companies may also make referrals. Patients or family members may also refer.

An Intake Coordinator will obtain information required for the admission process.
