St. John’s Outpatient Diabetes Self-Management Program
Diabetes affects an estimated 17 million people in the United States. A leading cause of heart disease and stroke, kidney disease, blindness and circulatory problems, diabetes can also lead to nerve damage, dental disease and sexual dysfunction.
St. John’s Diabetes Self-Management Program is designed to educate individuals and to teach them skills necessary to manage their diabetes. Education services include self-management classes and support groups. A team approach is used with YOU the patient, your physician, a St. John’s registered dietitian, an RN and a social worker.
St. John’s Outpatient Diabetes Self-Management Program is:
- Recognized by the American Diabetes Association for quality education
- The first in the four-state area
- A program that empowers patients and their families with knowledge and skills to effectively manage their diabetes
Adult Diabetes Support Group
- Free and open to the public
- Meets second Thursday of each month from September through May
