For copies of or to view your medical record contact the Health Information Management (Medical Record) Department. The department is located on the 2nd floor of the main hospital, or call 625-2175. The hours for Release of Information are 8:00 a.m. to 5:00 p.m., Monday through Friday. Proper identification (photo ID) and signing an authorization are required. For records on anyone besides yourself (i.e. child, spouse, etc.), or if you are under the age of 18, please contact us to determine the necessary legal requirements for access. Charges for copies may apply.
