Hospice Team

St. John's Hospice team is a multi-disciplinary group of professionals and volunteers. Their participation in this very special program is more than a job, it is a calling. It requires that they work together to meet the complex needs of the patient and family, and that they do so with a great deal of love and compassion. The team works closely with consulting physicians and the hospice patient's attending physician to develop an individualized quality care plan.

Hospice registered nurses are experts at caring for the terminally ill. Their areas of expertise lie in pain management and symptom control.

Nurse aides are skilled in providing comfort services. These include bathing, shaving, hair care and other personalized assignments.

Social workers evaluate the financial, social and emotional status of the patient and family, as well as provide long-range planning, counseling and coordinate community services.

The St. John's Hospice chaplain, working closely with the patient's clergy, serves as a spiritual counselor to the patient and family.

Bereavement services to support families and caregivers are also provided by St. John's Hospice for 14 months following the death of your loved one.

Hospice volunteers assist families by performing such duties as transportation, companionship, meal preparation and letter writing.
