Program Participation
Patients can be referred to St. John's Hospice by anyone. After referral to the Hospice office, a hospice admissions nurse interviews the patient and family. The information gathered is then reviewed with the patient's physician to determine admission appropriateness.
Once entered into the St. John's Hospice program, patients receive care at home or in a nursing home. Hospice patients also may continue receiving services from their attending physician.
