Cardiac Rehabilitation Program
St. John's "Partners for Life" outpatient Cardiac Rehabilitation Program is a three phase program designed to counsel and educate patients with Heart Disease.
Phase I teaching is completed by the inpatient cardiac rehab nurse while the patient is in the hospital. Phase II is a 12 week, 36 session program that is designed for people who have had a Heart Attack, Bypass Surgery or Stable Angina (meeting criteria). These patients are monitored by telemetry on every session and have the opportunity to attend educational classes.
Phase III is our maintenance/wellness program which is designed for people with or without heart disease. We encourage our patients to transfer to this phase after completing Phase II to maintain a lifelong commitment of healthy living. We also encourage the participation of family members.
